Some of the mods just update existing games. This mod for Ecco the Dolphin, for example, adds the improvements made to the Japanese version of the game. The same approach was taken for this bug-free version of Phantasy Star II.
This is called “Streets Of Rage 2 Except It Makes That Weird Tim Allen Noise When People Die.” Upon launch, it was on the top of the charts for a while.

Sonic 3 Complete is currently the most popular mod. It’s a combination of Sonic 3 and Sonic & Knuckles (which were developed as one game, then split in two), and it adds a menu for easy access and customization the Genesis games never had.
Somewhere overseas, an illegal NES game was sold called “Somari” that placed Mario into Sonic’s world (and given it was technically a port of Sonic 1 for the NES, it was quite the effort for a pirate). Now that game has been recreated using Steam Workshop, which wasn’t hard…
